ProcDoc Terms of Service

By using ProcDoc, you agree to use the service in accordance with your professional, legal, and institutional obligations.

Data-Use Addendum & Attestation

ProcDoc stores a minimized, de-identified clinical dataset. It is not a HIPAA system of record and we do not enter into Business Associate Agreements for individual accounts. By accepting these terms you attest that:

  • You are responsible for your own HIPAA and institutional compliance obligations.
  • You will enter only permitted, de-identified data (see the Acceptable Use & Data Minimization policy).
  • You will never enter direct patient identifiers — names, MRNs, SSNs, full dates of birth, addresses, contact details, photos, account/insurance numbers, or any free text that names a specific person.
  • You acknowledge the safeguards ProcDoc provides (encryption in transit and at rest, row-level data isolation) and that they do not relieve you of your own obligations.

This attestation is a condition of using ProcDoc. Misuse — including entering prohibited identifiers — may result in suspension or termination of your account.
